Your service area is your office's county and the counties next to it (3345-1.0 "Service Area"). The manual prints it everywhere it is needed.

Do you own or manage any other agency or facility?

Part 1 Exhibit E lists them. If none, the manual prints "None" on the exhibit.

Are you claiming tax-exempt (not-for-profit) status?

Yes reserves a page for your proof of not-for-profit status (checklist row 2.2.2.3.4). No marks that row N/A in the index.

Office hoursThe director or a designee must be available to consumers at all times during the agency's operating hours (3345-3.9). DHCQ's review guide expects employees to be told the office hours and the days and hours care is available.

DHSS's own forms go on reserved pages

Some documents in the binder are DHSS's own forms: the Notice of Direct Care Worker Status each consumer receives, the Part 1 application for a new agency, and the RN/LPN Attestation if you employ a nurse. They are used exactly as DHSS publishes them, so your manual does not reproduce them. Each has a reserved page saying which form goes there, where to get it and what to fill in. You put the completed DHSS form in the binder in place of its reserved page, so the page numbers do not change.

  • The orientation program: 25 teaching modules, one for each of the 24 orientation topics in 3345-4.5.2, from the agency's structure and consumer rights to nutrition, infection control, body mechanics and dementia-specific training, and one on your disaster preparedness plan
  • The competency test: 80 multiple-choice questions covering all 25 modules, with the answer key on its own page in the same order as the test, and the pass mark you choose
  • The modules and the test are training material written from cited public sources (CDC, OSHA, NIA and others) and Delaware's rules, not reviewed by a nurse, trainer or DHCQ. Each module lists its sources, and your director reads every module before you adopt it
  • Consumer services: intake, the initial home visit and consumer evaluation, the consumer agreement, service plans and follow-up home visits, what direct care workers may and may not do, transportation, services in a hospital or other licensed facility, and case closure
  • Medication: reminders only by default, with the medication attestation DHCQ asks for. If you choose medication administration authorized by a responsible caregiver under 3345-5.4.3, its policy and forms, and the rule that no worker gives medication before completing a Department approved training program
  • Records, grievances and emergencies: consumer records and activity logs, retention, computerized records, confidentiality and release of information, consumer grievances, and a comprehensive emergency management plan
  • Written in non-medical words, as DHCQ's review guide asks (3345-3.13 keeps health care language out of your agency's name and advertising). Nothing in it offers nursing or other health care services
  • Already licensed? Policy 1.1 ends with a renewal and annual review checklist that follows DHCQ's renewal application (its survey questions, Exhibits A to J and the attestation) and then lists the yearly duties in rule 3345, each with the policy that answers it
  • A Document Compliance Checklist index as its own Word and PDF file: all 57 rows in DHCQ's order and words, each with the pages of your PDF manual to type into DHCQ's own workbook (DHCQ has not approved it)
