Tennessee PSSA license: cost and timeline

TDMHSAS charges $810 per site for a Personal Support Services Agency license, with the initial application and with every renewal, and the fee is non-refundable. Neither the rules nor TDMHSAS publish a processing time. Here are TDMHSAS’s fees, the other costs with official prices, each step from the four forms to the initial license, renewal, and what the proposed rewrite of the licensing rules would change.

Quick answer

Cost: the Tennessee Department of Mental Health and Substance Abuse Services (TDMHSAS) charges $810 per site for one category of service, the PSSA license, with each initial and renewal application (0940-05-02-.05(1)). Other costs with an official price: TBI’s fingerprint checks ($37.15 for an applicant through a qualified organization, $50 for an FBI-only check for the general public) and DDA’s optional medication administration class ($50 per participant). Liability insurance is required, but no minimum is set and we found no official price.

  • Unpaid fees hold up the license. TDMHSAS may withhold a license, or suspend or expire an existing one, until fees are paid (0940-05-02-.23(3)).
  • Fee rule last amended in 2019. The fee rule was last amended by rules effective June 27, 2019 (0940-05-02-.05, history), and TDMHSAS’s proposed rewrite keeps the $810 fee (proposed 0940-05-02-.05).

Working out your TDMHSAS fees

Simple arithmetic, not a TDMHSAS figure

Your first two license years. One site, one category: the initial fee and one renewal, $810 + $810 = $1,620 at current rates (0940-05-02-.05(1)).

A second site. Each site has its own license and its own fee, so two PSSA sites pay $810 × 2 = $1,620 a year at current rates. A second category at the same site instead raises that site’s fee to $1,010, $200 more (0940-05-02-.09).

Our reading of the rules

No fee for the yearly inspection. The $50 maximum is for onsite inspections other than the initial or annual one, which are charged at the license fee rate, so by our reading a year with only the routine inspection costs the license fee alone (0940-05-02-.23(1)).

Other costs of getting and keeping the license

Each card says where the duty comes from. We give a price only where an official source sets one; none of the sources we checked publishes prices for insurance, private investigator checks or a nurse trainer.

The background check of the person responsible

The application asks for a TBI fingerprint check of the person with overall responsibility (or FBI fingerprints, or a licensed private investigator check covering current and previous states), and the individual pays for it. TDMHSAS does not publish the amount, and we have not confirmed which of TBI’s services the application expects. TBI lists $50 for an FBI-only (nationwide) fingerprint check for the general public, through its contractor; these are TBI’s fees.

Worker criminal checks: $37.15 each through TBI, or a private investigator

Fingerprints are needed if TBI or the FBI runs the check; a Tennessee-licensed private investigation company can run it without fingerprints. For qualified organizations, TBI lists $37.15 for a Tennessee and FBI fingerprint check of an applicant (TBI’s fee). TBI says qualified agencies, such as those serving the elderly, may request an ORI number, which sets up fingerprinting of prospective employees under a TBI user agreement. TBI’s $29 Tennessee name search uses no fingerprints, so by our reading it is not the TBI route TDMHSAS describes.

Abuse Registry and Sex Offender Registry checks

Every worker is checked on both registries before direct contact and every year after. The Abuse Registry is searched online through the Health Facilities Commission, and the Sex Offender Registry is TBI’s. We found no fee stated on either registry page we saved.

Liability insurance

Proof of liability insurance for the agency, its workers and others who provide services. No minimum amount is set. If workers drive clients, vehicle liability insurance too. We do not quote premiums.

Medication administration certification: $50 per person (DDA’s fee)

Optional. For PSSAs not serving people with intellectual or developmental disabilities, DDA charges $50 per participant for the 20-hour class and $25 for the Test-Out renewal; certification lasts 3 years. From a third attempt at a test, DDA bills the agency $5 for a test-out and $50 for class testing (DDA’s charges). TDMHSAS says only people working for agencies licensed by TDMHSAS as a PSSA, or meeting other eligibility criteria, qualify, so by our reading this cost comes after the license.

A registered nurse, if you offer medication assistance

Workers who give medication assistance need documented training by, or under the general supervision of, a registered nurse. An agency that does not offer medication assistance or education services needs no nurse for the license. We found no official price.

Business registration and a certified charter

A corporation sends a copy of its charter certified by the Tennessee Secretary of State with the application. The Secretary of State’s own fees apply; we have not checked them.

How long it takes, step by step

The order follows TDMHSAS’s process page and the rules; only the steps with a fixed time show one. Tick steps off as you go; progress is saved in this browser only.

  1. Check which department licenses you

    Before applying

    TDMHSAS if 50% or more of your clients will be aged, or have a mental illness or substance abuse diagnosis; the Department of Disability and Aging if 50% or more will have an intellectual or developmental disability. You attest to the mix with the application.

  2. Set up the business and the person responsible

    Before applying

    The person with overall responsibility needs proof of citizenship or legal immigration status, a fingerprint or licensed investigator background check they pay for, and three character references. A corporation sends its charter certified by the Secretary of State.

  3. Write and adopt your policies

    Before applying

    The desk audit covers certain policies and procedures, and deficiencies are corrected before a license is issued. The 17 policy items and 8 personnel topics.

  4. Send the four forms to your regional office

    Day 0

    The Initial Application, a Fact Sheet for each location, the Background Check and Privacy Statement form and the Financial Statement, to the West, Middle or East regional office. The rule says you should also give the counties you will serve and your records address; our suggestion is a letter sent with the forms. The forms item by item.

  5. Receive and pay TDMHSAS’s $810 invoice

    No published time

    TDMHSAS sends an invoice once it has the forms. Send a copy of the invoice with the payment, or the fee may not be credited to your application.

  6. The electronic desk audit

    No published time

    Done after the paperwork is in and the fee is paid. A Regional Manager can explain the next steps.

  7. Correct any deficiencies

    Before any license

    TDMHSAS tells you of any deficiencies, and they must be corrected before a license is issued. It may allow a reasonable time to correct deficiencies found by inspection.

  8. Initial license, then services may start

    Up to 12 months

    An initial license lasts no more than 12 months. Post it where the public can see it. No services before it is issued. If TDMHSAS denies the application instead, you have 15 calendar days to ask in writing for a hearing.

  9. Hire, check and train each worker before they meet a client

    Every hire

    Background check results back before any direct contact, as TDMHSAS reads the statute, plus registry checks, references and competency. DDA’s medication administration classes are for staff of agencies already licensed as a PSSA, or meeting other eligibility criteria.

  10. Unannounced inspection

    Every year

    At least one a year, on dates that vary from year to year.

  11. Renew when TDMHSAS writes

    Every year

    Before the license expires TDMHSAS tells you what to send, with the fee by invoice ($810 at current rates) and a new client-mix attestation. If every service is in compliance, a full license follows.

Why we give no date

TDMHSAS publishes no processing time, range or average:

  • No processing time. Neither the rules nor TDMHSAS’s pages give a processing time from application to license (checked October 2, 2026). TDMHSAS says: "A Regional Manager can provide an in-depth explanation of the next steps." (Become a Licensed Provider, step 5)
  • Waits at three points. The invoice comes once TDMHSAS has the forms (step 3), the desk audit follows the paperwork and the fee (step 5), and any deficiencies are corrected before a license, in what TDMHSAS may allow as "a reasonable time period" (0940-05-02-.11).
  • The only fixed periods are ones you must meet: 15 calendar days to ask for a hearing after a denial (0940-05-02-.06(5)(c)), 30 days to pay an extra inspection invoice (0940-05-02-.23(2)), and the date TDMHSAS sets on a Notice of Non-Compliance for your Plan of Compliance (0940-05-02-.21).

We have no basis for an estimate, so we do not make one. Our suggestion: when you send the forms, ask the Regional Manager what to expect (TDMHSAS regional contacts).

What you control: complete, legible forms with every item answered (0940-05-02-.04(2)), paying the invoice promptly with a copy of it, and every required policy written and adopted before the desk audit. Our suggestion: do not sign client contracts or set a start date until the license is issued. No services may begin before TDMHSAS issues it, and unlicensed operation is unlawful and may bring civil or criminal sanctions (0940-05-02-.03(1)).

Renewal and changes after you are licensed

  • Term. "Initial licenses will not exceed 12 months." (0940-05-02-.02(1)) A full license is valid for up to 12 months from issue (0940-05-02-.02(2)). "The expiration date of all licenses issued by the Department will be indicated on the face of the license." (0940-05-02-.15)
  • TDMHSAS starts the renewal. "Prior to the expiration of an initial, full or provisional license, the Department will notify the licensee of the need to submit an application for license renewal." It tells you the information, fees (by invoice) and documents to send (0940-05-02-.07(1)). Each renewal includes a new client-mix attestation (0940-05-38-.04(4)).
  • On time keeps you licensed. If you file a timely and complete renewal with the fees, "the existing license does not expire until the status of the application has been determined by the Department" (0940-05-02-.15).
  • Full or provisional. At renewal TDMHSAS will "review the application material, the current life safety and environmental conditions, when applicable, and the performance history of the facility or service"; a full license follows if every service is in compliance, and a provisional license may be issued if not (0940-05-02-.07(5)). TDMHSAS may issue a provisional license to an agency not meeting every requirement, if it is making a diligent effort, clients are not endangered, it has an acceptable compliance plan and it met last year’s plan. (0940-05-02-.02(3))
  • Inspections. At least one unannounced inspection a year (0940-05-02-.22), and "Dates for unannounced inspections will be random varying from year to year." (0940-05-02-.07(4)) How inspections work.
  • Selling or moving. Licenses cannot be transferred. A new application and license are needed before services are provided after a change of ownership or location. (0940-05-02-.10) For a move, TDMHSAS says a new license is needed before the relocation, with a Fact Sheet for the new location and a Change of Status form closing the old one (TDMHSAS Provider FAQs).
  • Other changes. A new chief executive or executive director (with background forms and fingerprinting), a name change or a closure goes on the Change of Status form (MH-4454). The certificate is surrendered on closure or a change of owner (0940-05-02-.17).

What the proposed rewrite would change for cost and timing

On September 24, 2026 TDMHSAS filed proposed amendments rewriting every rule in Chapter 0940-05-02. The $810 fee stays the same (proposed 0940-05-02-.05). The changes that touch cost and timing:

  • Fees before the initial inspection. No initial inspection until the site’s fees are paid and registered; fees follow an invoice. Amounts unchanged ($810 for one category). (Proposed 0940-05-02-.05(2))
  • License types. A site not yet serving clients would renew to another initial license; a full license needs substantial compliance; a provisional license lasts up to 12 months and has the same force as a full license. (Proposed 0940-05-02-.02(1)-(3))
  • Renewal extensions. TDMHSAS may extend a license 60 days while a timely renewal is decided, then monthly on written request. (Proposed 0940-05-02-.15(2)-(3))
  • Overdue Plans of Compliance block renewal. A license would not be renewed while any Plan of Compliance is overdue. (Proposed 0940-05-02-.07(6))
  • Civil penalty notice in writing. Staff would notify a licensee in writing (not verbally) that a civil penalty may be imposed. Amounts unchanged. (Proposed 0940-05-02-.25(2)(a))

By our reading, the rule on fees before the initial inspection matches TDMHSAS’s process page today, so for a new agency the practical change is small: pay the invoice promptly. Every proposed change.

Civil penalties, per violation, per day

TDMHSAS may impose civil penalties for each violation, per day: $500 for a first Priority 1 or critical offense and $250 for other first offenses. A second offense of the same type within 12 months can bring $2,500 to $5,000 (Priority 1 or critical) or $500 to $2,500 (other offenses).

Myths about cost and timing

Is TDMHSAS’s $810 PSSA license fee a one-time payment?

No. The fee goes with each initial and each renewal application, and a license lasts no more than 12 months, so by our reading you pay it every year. It is also per site, because TDMHSAS issues one license for each site. It is TDMHSAS’s fee, not a CareRulebook price.

Does TDMHSAS bill $50 for every inspection, including the yearly one?

No. The fee of up to $50 is for onsite inspections other than the initial or annual inspection; those are charged at the license fee rate in 0940-05-02-.05. By our reading the yearly inspection is covered by the yearly license fee. Any other inspection fee is invoiced and due within 30 days.

Can I start serving private-pay clients while my PSSA application is pending?

No. No one may begin delivering services until TDMHSAS issues the license, and providing personal support services without a license is unlawful. TDMHSAS issues an initial license (up to one year) after its desk audit and after any deficiencies are corrected.

How much does a PSSA license cost in Tennessee?

TDMHSAS’s fee is $810 per site for one category of service, sent with the initial application and again with every renewal. A site licensed for two categories pays $1,010. Do I get the fee back if TDMHSAS denies my license?

No. The fee is paid before TDMHSAS decides whether to grant or deny the license, and all fees submitted are non-refundable. A denied applicant may ask in writing, within 15 calendar days, for a hearing before the Licensure Review Panel.

How much do background checks cost for a Tennessee PSSA?

TDMHSAS does not publish an amount. A worker’s check can be run by TBI or the FBI with fingerprints, or by a Tennessee-licensed private investigation company without fingerprints. TBI lists $37.15 for a Tennessee and FBI fingerprint check of an applicant for qualified organizations, which need an ORI number from TBI, and $50 for an FBI-only fingerprint check for the general public. These are TBI’s fees. We found no official price for private investigator checks.

Can I serve clients while I wait for the license?

No. No one may begin delivering services until TDMHSAS issues a license, and providing personal support services without one is unlawful and may bring civil or criminal sanctions.

Will the fee change?

Not under TDMHSAS’s current proposal. Its rewrite of Chapter 0940-05-02, filed September 24, 2026 with a hearing on November 18, 2026, keeps the $810 one-category fee, says fees follow an invoice, and says no initial inspection is scheduled until the fees are paid. It is a proposal and is not in force.
