Home care license cost by state

What each state charges to license a non-medical home care agency: the first fee, renewals, late fees, bonds, background check fees and other charges. Every amount is a state fee (or, where the cell says so, a fingerprint vendor’s charge), cited to the official rule or regulator page, never a CareRulebook price.

Quick answer

To apply for and receive a first license, at the smallest fee tier, the state’s fees run from nothing in Maryland to $5,000 in Washington. The lowest fee charged is $250, in Indiana, Kansas and Ohio. For the first two years, counting the fees that cover the first 24 months of one license at the smallest fee tier (our arithmetic from the cited fees), the state’s fees run from $250 in Ohio to $7,400 in Washington, or nothing in Maryland (our reading). These are state fees, not CareRulebook prices, and they change, so check the regulator’s current fee before you pay.

Licenses last three years in Ohio, Texas and Virginia. Georgia and New York have no expiry date but a yearly obligation: Georgia, $250 a year per service; New York, yearly registration. By our reading, licenses no longer expire in Maryland. Colorado, Illinois, Indiana, Kansas, Nevada, North Carolina, South Carolina and Tennessee renew every year, and in Washington a first 12-month license is followed by two-year licenses.

A surety bond is required in Nevada ($5,000 to $50,000 surety bond) and Ohio ($20,000 surety bond). Colorado and South Carolina ask for liability insurance or a surety bond: at least $100,000 per occurrence, $300,000 aggregate. Liability insurance minimums are set in Illinois ($1 million per occurrence, $3 million aggregate), Virginia ($1 million liability, $50,000 crime cover, plus malpractice) and Washington ($1 million per occurrence). Other bond or insurance rules: Georgia (only an employee bond, if staff handle client funds), Indiana (only for lift services) and Tennessee (liability insurance, no amount set). Background check fees go to a state agency or its fingerprint vendor; our sources give an amount for Colorado, Maryland, South Carolina and Tennessee.

How to read the table

  • Every amount is a state fee. It is what the state regulator, or another state agency named in the cell, charges under its rule or fee schedule (or, where the cell says so, what that agency’s fingerprint vendor collects). CareRulebook’s manual price is separate and appears nowhere in this table.
  • Fees change. Some states set new fees every year. Each row shows the date we checked the source; confirm the current fee with the regulator before you pay.
  • “Our arithmetic” marks the two-year figure. No state publishes it: we add the cited fees that cover the first 24 months of the license, for one license at the smallest fee tier, paid on time, and say what we assumed.
  • “Our reading” marks an answer that is our interpretation, because the rule does not say it in so many words. Confirm those with the regulator.
  • “Not covered in our sources” means the official texts we saved do not answer the question. It does not mean there is no fee.
  • Not the whole cost of starting. Insurance and bond premiums, business registration, office, training and wages are extra.

Colorado

How much does a home care license cost?

It depends on the state. To apply for and receive a first license, at the smallest fee tier, the state’s fees run from nothing in Maryland to $5,000 in Washington. The lowest fee charged is $250, in Indiana, Kansas and Ohio. For the first two years, counting the fees that cover the first 24 months of one license at the smallest fee tier (our arithmetic from the cited fees), the state’s fees run from $250 in Ohio to $7,400 in Washington, or nothing in Maryland (our reading). These are the fees each state charges, not CareRulebook prices.

Which state has the cheapest home care license?

Maryland: OHCQ charges no fee to apply for an RSA license, although COMAR 10.07.05.04 still lists $1,000. Among states that charge a fee, the lowest initial fee is $250, in Indiana, Kansas and Ohio. Indiana: $250 a year to renew; Kansas: $100 or $350 a year to renew; Ohio: $250 every three years to renew.

Which state has the most expensive home care license?

Washington, at $5,000 to apply for and receive a first license. Per service category, for the first 12-month license (DOH’s fee since June 1, 2026). The first county is included. How often do you renew a home care license?

Licenses last three years in Ohio, Texas and Virginia. Georgia and New York have no expiry date but a yearly obligation: Georgia, $250 a year per service; New York, yearly registration. By our reading, licenses no longer expire in Maryland. Colorado, Illinois, Indiana, Kansas, Nevada, North Carolina, South Carolina and Tennessee renew every year, and in Washington a first 12-month license is followed by two-year licenses. Check each state’s row for the renewal window.

Do you need a surety bond to start a home care agency?

A surety bond is required in Nevada ($5,000 to $50,000 surety bond) and Ohio ($20,000 surety bond). Colorado and South Carolina ask for liability insurance or a surety bond: at least $100,000 per occurrence, $300,000 aggregate. Liability insurance minimums are set in Illinois ($1 million per occurrence, $3 million aggregate), Virginia ($1 million liability, $50,000 crime cover, plus malpractice) and Washington ($1 million per occurrence). Other bond or insurance rules: Georgia (only an employee bond, if staff handle client funds), Indiana (only for lift services) and Tennessee (liability insurance, no amount set). For Kansas, Maryland, New York, North Carolina and Texas, our sources do not cover a bond or insurance requirement. Bond and insurance premiums are paid to a surety or insurer and are not state fees, so they are not in the totals.

How much do background checks cost for a home care agency?

Where our sources give an amount: Colorado, $39.50 (CBI fee); Maryland, $38 (DPSCS fee); South Carolina, $30.25 (paid to DPH’s fingerprint vendor); Tennessee, $37.15 (TBI fee). Elsewhere our sources say who pays but not how much: Georgia, set by DCH, amount not stated; Illinois, fee set by the State Police, no amount in the Act; Indiana, agency pays, amount not in our sources; Kansas, KDADS fee, current amount not confirmed; Nevada, license applicants pay; the agency pays for staff; New York, not charged to workers, amount not in our sources; Ohio, the agency pays BCI, amount not in our sources; Washington, DSHS pays for caregiver checks. For North Carolina, Texas and Virginia, our sources do not cover the check fee. Fingerprinting vendors often add their own fee on top. These fees belong to state agencies or their vendors, not CareRulebook.

What happens if you renew a home care license late?

Colorado: 10% to 75% of the renewal fee; Georgia: $150; Illinois: a possible fine; Indiana: a possible civil penalty (up to $1,000); Kansas: automatic cancellation; Maryland: no renewal, so no late fee (our reading); Nevada: renewal fee plus half (compiled rule); New York: $500 a month; North Carolina: a gap in licensure; Ohio: possible license action; South Carolina: $200; Tennessee: no late fee in the rules we saved; Texas: half the renewal fee; Virginia: $50; Washington: $50 a day, up to $1,000. Are these fees the whole cost of starting a home care agency?

No. The table covers the fees state agencies charge. It leaves out insurance and bond premiums, business registration, office costs, training, wages and any consultant or software you choose. CareRulebook’s manual price is separate and is not included.

What does “Our arithmetic” mean?

The two-year figure is not a number any state publishes. We add the cited state fees that cover the first 24 months of the license: the first license plus the renewals needed for the rest of the 24 months, for one license at the smallest fee tier, paid on time. Each row says what we assumed, and gives a range where the start date or the length of the first license changes the count. Fees change, so check the current fee with the regulator before you budget.

What does “Not covered in our sources” mean?

It means the official texts we saved and verified for that state do not answer the question. It does not mean the answer is no or that there is no fee. We leave the cell empty rather than guess, and add the fact once we can quote it from an official source.

How current is this table?

Each row shows the date we last checked that state’s rules against the official text (between September 30, 2026 and October 3, 2026). States change fees, some every year, so confirm the amount with the regulator before you pay. We log every correction on our corrections page.
